Chan, a 6-month-old boy presents with poor weight gain.

Feeding history

• Exclusively breastfed since birth
• Feeding time is progressively longer in past 3 months
• Feeds for 30 minutes, has excessive sweating and pauses during feeding
• Often sleeps for 30 minutes, then would wake up for feeds again
• Seems breathless during feeding, but does not have cyanosis

• No history of antenatal infection and use of medication
